What you will need to provide
- Contact and emergency contact details
- Complete medical history — conditions, surgeries, allergies
- All current medications, including over-the-counter drugs and supplements. This genuinely matters: blood thinners, bisphosphonates, and several common supplements affect how we plan treatment and sedation
- Insurance information, if applicable
- Consent and privacy acknowledgements

Transferring from another dentist?
Ask your previous office to forward recent x-rays and records. It avoids repeat imaging and gives us your history rather than a snapshot.
